Healthy Workplace Awards Program – Sponsored by Blue Cross of Northeastern Pennsylvania and ChamberChoice

Judging Criteria: The program will recognize companies that have adopted wellness practices in the workplace. Nominees will be judged on the following five questions, which match the goal and objective of this award - successful company workplace wellness.
• How many employees do you have? (Awards will be given in 2 categories 2-50 and 50+ employees)
• How many employees participate in your wellness program?
• What is your organization doing to promote a healthy workplace?
• What incentives do you offer to your employees?
• What are the results of your workplace wellness program?

Nomination Submission Instructions: Companies can nominate themselves or be nominated by a third party. Nominees will be evaluated on the strength and creativity of their workplace wellness initiatives.

The award will be presented at the Annual Membership Luncheon.

All Nominations are due by Friday, September 19, 2008
